Note to Applicants

Admission to the SOHL Good Times Tournament is open to players currently 39 years of age or older. Tournament participation will be determined by a combination of the date applications are received, previous applications to join the SOHL & previous tournament participation. Applications and cheques not processed for the tournament will be destroyed and will not be returned to the applicants. An application is not considered as received unless it is accompanied with the entrance fee.

ASSUMPTION OF RISK
RELEASE AND WAIVER OF LIABILITY
INDEMNITY AGREEMENT

By participating in the SOHL Good Times Tournament, and for good and valuable consideration, the participant hereby agrees to release and indemnify, and save harmless the said SOHL Executive, Southdale Recreation Association and players in the SOHL Good Times Tournament and anyone acting on their instructions against all actions, losses, costs, charges, damages and expenses which I or my estate may at anytime hereafter sustain, suffer or be put to by reason of the performance of their lawful authorized duties as members of the executive of, or by virtue of their positions in the said league, or anytime in any matter relating to the SOHL Good Times Tournament. I further acknowledge that I understand that even Oldtimers Hockey carries with it the risk of injury, and I hereby acknowledge that I have personally accepted that risk and forever release the Executive or persons acting on their behalf from that risk.

I HAVE READ THIS DOCUMENT THROUGHLY.

I UNDERSTAND THAT THE RELEASEES ARE RELYING UPON MY WARRANTIES, ASSUMPTIONS, WAIVER AND RELEASE, UNDERTAKINGS AND AGREEMENTS WHEN ACCEPTING MY PARTICIPATION IN THIS PROGRAMME.

I UNDERSTAND THAT BY SIGNING THIS DOCUMENT I GIVE UP SUBSTANTIAL LEGAL RIGHTS I WOULD OTHERWISE HAVE.

I SIGN THIS DOCUMENT VOLUNTARILY AND WITHOUT INDUCEMENT.
